Full-time employees receive:

Eligibility
When benefits begin:
- Full-time employees: First day of month after 30 days
- Part-time employees: Not eligible
- Dependents: Spouse, children under 26
- Domestic partners: Eligible with proof
Initial Enrollment
New employees must enroll:
- Within 30 days of hire date
- Log in to benefits portal
- Review plan options
- Select coverage levels
- Add dependents
- Designate beneficiaries
- Submit enrollment
If you miss deadline, must wait until next open enrollment.
Medical Insurance
Plan Options:
- PPO Plan: More flexibility, higher cost
- HMO Plan: Lower cost, network required
- HDHP + HSA: High deductible, tax-advantaged savings
Coverage Tiers:
- Employee only
- Employee + Spouse
- Employee + Child(ren)
- Family
Premium cost-sharing varies by plan and tier.
Dental Insurance
Two plan options:
- Basic Plan: Preventive care, basic procedures
- Premier Plan: Includes major procedures, orthodontics
Coverage includes:
- 2 cleanings per year (100%)
- Fillings (80%)
- Crowns, root canals (50%)
- Annual maximum: $1,500
Vision Insurance
Annual benefits:
- Eye exam (100% covered)
- Lenses or contacts
- Frames allowance ($150)
- Discounts on LASIK
401(k) Retirement Plan
Building your future:
- Eligibility: Immediate
- Employee contribution: Up to IRS limit
- Company match: 50% of first 6% contributed
- Vesting: Immediate
- Investment options: Target date funds, index funds, etc.
Enroll or change contributions:
- Log in to Fidelity.com
- Select contribution percentage
- Choose investments
- Changes effective next pay period
Life Insurance
Basic Life:
- Company-paid: 1x annual salary
- Automatic coverage
- No cost to employee
Supplemental Life:
- Employee: Up to 5x salary
- Spouse: Up to $250,000
- Child: Up to $10,000
- Employee-paid
- Evidence of insurability may be required
Disability Insurance
Short-Term Disability:
- Company-paid
- 60% of salary
- Waiting period: 7 days
- Maximum: 26 weeks
Long-Term Disability:
- Company-paid
- 60% of salary
- Begins after STD exhausted
- Until age 65 or recovery
Flexible Spending Accounts
Healthcare FSA:
- Up to $3,050 per year
- Pre-tax dollars
- For medical expenses
- Use it or lose it (carryover $610)
Dependent Care FSA:
- Up to $5,000 per year
- For childcare or elder care
- Must have qualifying dependent
- No carryover
Health Savings Account (HSA)
With HDHP plan only:
- Employee contribution: Up to $4,150 (individual), $8,300 (family)
- Company contribution: $500 (individual), $1,000 (family)
- Funds roll over year to year
- Portable if you leave
- Triple tax advantage
- Investment options available
Open Enrollment
Annual benefits review:
- Held each November
- Changes effective January 1
- Must actively enroll to make changes
- If no action, current coverage continues
- Review FSA elections (don't auto-renew)
Qualifying Life Events
Changes allowed mid-year for:
- Marriage or divorce
- Birth or adoption
- Death of dependent
- Spouse job change
- Change in dependent status
- Move to different state
Must request change within 30 days of event.
Making Changes
- Log in to benefits portal
- Click "Life Event"
- Select event type
- Provide effective date
- Upload documentation
- Make benefit changes
- Submit
COBRA
Continuing coverage after employment:
- Available for up to 18 months
- You pay full premium + 2% admin fee
- Must elect within 60 days
- Contact HR for details
